Referral is received and an assessment to be carried out within 48 hours by 2 members of the multi-disciplinary team.

Assessment reports are produced and discussed in the multi-disciplinary team and decision whether to admit or not is made and communicated to the referrer and a copy of the report is sent too.

Upon admission Multi-disciplinary team assessments, formulations and discharge plans are devised. As a direct result of the assessments and formulations a wide range of therapies are offered according to clinical need.

Multi-disciplinary treatment and rehabilitation models are implemented with regular reviews and CPA while discharge planning continues through out.

Discharge into step down facilities, transitioning to community through social inclusion, involving the Community Mental Health Teams.
